Why HLA-B27 Test?

CLINICAL INFORMATION

In general, the presence of HLA antigens on the nucleated cell surfaces in our body enables the immune system to recognize and distinguish between the self-antigens present on the normal cell surfaces, against the invading (non-self) foreign pathogens such as bacteria etc. Each and every person have a number of different types of these proteins on each cell surface and thus together they make up an individual’s “Tissue Type” and one such HLA protein is called HLA B27. This HLA B27 (Human Leucocyte Antigen) is a protein found on the surface on the WBC surfaces in some rare individuals and in such cases, the presence of HLA-B27 is associated with a higher risk of developing certain autoimmune disorders such as ankylosing spondylosis and/or its associated seronegative spondylo-arthropathies – eg. uveitis (isolated eye disorder such as anterior uveitis), reactive arthritis (Reiter syndrome) and/or juvenile rheumatoid arthritis (JRA), IBD, Psoriatic arthritis, Bechet’s disease (all these conditions fall under the umbrella of seronegative spondyloarthropathies) etc. This test is indicated in suspected patients (who could be at a high risk of developing autoimmune diseases) with clinical manifestations (signs and symptoms) that include acute or chronic inflammation associated with pain in the spine, neck, chest, eyes and joints. Hence a positive HLA B27 test result is suggestive that the patient could be prone to ankylosing spondylosis (an autoimmune disease or its associated disorders) hence the early diagnosis can prevent further complications. Other additional tests include CBC, karyotyping, tissue or immune typing, Molecular diagnostics, imaging studies etc.
